Pityriasis Lichenoides Chronica. Dermpath Made Easy.

Pityriaisis Lichenoides Chronica - can you say that again? Pretty sure this was what I said when I first learned about this lesion. Pityriasis- Hippocrates description of the scruffy appearance of the skin that looked like it was covered by the fine bran of grain called "pityron." Lichenoides: no definition listed on google, but I assume it's the pattern of inflammation - lichenoid, band-like inflammation at the dermoepidermal junction. Chronica: an older lesion with less intense inflammation and changes (compared to PLEVAcuta) Multiple red-brown scaly papules of uncertain cause. Sometimes confused with vasculitis. Microscopically: 1. In PLC the inflammatory infiltrate is largely lymphocytes and some macrophages and is more superficial than PLEVA (PLEVA-superficial and deep and wedge-shaped) 2. The changes are milder (less necrosis, less RBC extravasation) 3. Confluent scale 4. Lymphocytes along DEJ (Lymphocytes in every hole) 5. RBC extravasation present, albeit less than PLEVA Main points: Clinically multiple papules + Microscopically - licheoid inflammation (lymphocyte in every hole) + overlying hypekeratosis and parakeratosis + red cell extravasation + dyskeratosis + vacuolar change with melanin dropout #dermatology #skin #health #medical #medstudent #science #histology #pathology #dermpath If you have benefited from this video and want to see more, please hit that like button and share it with others.
