I was diagnosed with severe dysplasia and was recommended a cone biopsy. I just had the cone biopsy in February of this year (2025). I had heard that if the margins are positive, a simple hysterectomy is also an option and that this diagnosis has meaning, but I was skeptical because I was told that there is a little CN3 within CN1-CN2 to begin with, so I went to the cone biopsy. However, I was told that the margins were positive and that the severe dysplasia had extended slightly toward the uterus, so a simple hysterectomy was recommended. I wasn't convinced, so I went to a clinic in Tokyo, and during the internal examination, I was told that the cone biopsy was performed shallowly. For people of this age (postmenopausal), severe dysplasia has the tendency to extend toward the uterus, so if it is not removed for a long time, it is natural that the margins will be positive, and if it is not removed completely, a total hysterectomy would be necessary. As the doctor said, even if it is severe dysplasia, you don't need to worry too much about it, so you should have a checkup, an HPV test, and monitor the progress. I was scared to think about the discomfort and sense of loss that would follow after a total hysterectomy.
