Thank you for watching. 〇Definitions of Bipolar Disorder Type I and Type II • Type I is characterized by a clear manic episode. • A manic episode is a state characterized by grandiose delusions (feelings of omnipotence, omnipotence) lasting for more than a week, reduced sleep duration, excessive talkativeness, jumping between topics, distractibility, hyperactivity, and deviant behavior (extravagance, sexual deviance, etc.). • Type II is characterized by a less pronounced manic episode, often a mild manic state that is difficult for others to notice. 〇Neuroscientific Differences • Studies using functional MRI have shown that when viewing images representing hostility, Type I individuals exhibit an overreaction of the amygdala, while blood flow to the prefrontal cortex, which controls emotions, does not increase, indicating a lack of restraint. • Type II individuals tend to have a weaker amygdala response, a duller prefrontal cortex response, and generally lower responsiveness. Clinical Differences and Treatment • Type I is characterized by impaired social functioning (work, housework, studies, etc.) due to manic episodes. • The main treatment for Type I is controlling impulsivity and anger, and managing manic episodes. Mood stabilizers such as lithium and, if necessary, antipsychotics are used. • Type II is characterized by a predominantly depressive state and reduced activity. • Treatment for Type II primarily involves mood stabilizers, and approaches such as no-training to enhance cognitive flexibility (improve situational judgment) are also considered important. • Prevalence and Difficulty in Diagnosis • Type I is estimated to affect about 1% of the total population, while Type II affects about 1-1.5%. • Type II, in particular, is often misdiagnosed as depression because manic episodes are less prominent, and many cases are thought to be overlooked. For diagnosis, it's crucial for the doctor to elicit detailed information from the patient about specific behavioral changes (e.g., feeling fine with less sleep than usual, overspending, becoming more talkative, etc.).
