This video explains De Lee's planes in labor. The maternal pelvis has diameters that form a canal delimited by the superior, middle, and inferior straits, with the pelvic pouch between them. The superior strait is not of interest for De Lee's plane. The middle strait is at the level of the ischial spines (two bones). Remembering this, we can better understand the locations of the conjugates important in obstetrics and learn how to perform a clinical evaluation of the woman's pelvis. The clinical evaluation of the pelvis is important to determine whether or not there will be passage for the fetus through the pelvis. The so-called anatomical conjugate vera is the measurement from the superior border of the pubic symphysis to the promontory, approximately 11 cm. The obstetric conjugate vera, or diameter, is the measurement from the posterior aspect of the pubic symphysis to the promontory, measuring 10.5 cm. It is the diameter that the fetus actually has to pass through and is a very important measurement for the clinical evaluation of the pelvis. By touch, we can get an idea of this measurement, but not certainty, because in the vast majority of women, the touch does not reach from the promontory to the posterior face. And when the promontory is reachable - there is a suggestion that it may indicate problems for the mechanism of labor. Larger diameters of the presentation - larger diameters of the pelvis, that is, when the fetus is positioned in the best possible way to accommodate the largest diameter of its presentation with the largest diameter of the superior strait. The mother reports that her "belly has dropped". Due to uterine contractions, the cephalic pole (head of the fetus) is pressed against the pelvic structures and generates flexion movement (tilting movement), which reduces the diameter of its presentation and facilitates delivery. The diagnosis of flexion can be made by vaginal examination (the nurse feels the small fontanelle). DeLee's Planes = AM = high and mobile -3 = high but fixed -2 = high but descending -1 = almost at plane 0 0 = at plane 0 +1 = has already crossed the pelvis – in the mid-strait +2 = almost emerging +3 = emerging. *If you liked this video, please subscribe to my channel.
