Webinar Recording: Making Survivorship Care Plans Work For Patients: A Collaboration With SOMC

Recording of the webinar titled "Making Survivorship Care Plans Work for the Patient: A Collaboration Between CliniSync & SOMC's Cancer Center" that was recorded on 6/23/2021. The CliniSync HIE partnered with the Southern Ohio Medical Center’s (SOMC) Cancer Center to look at Survivorship Programs and the use of Survivorship Care Plans. This webinar took a closer look at how information could flow electronically to and from a cancer center for treatment and follow-up. This webinar also included insights from a recently reviewed accredited organization about the Commission on Cancer’s (CoC) new focus on Survivorship Programs: Purpose of the Webinar: o To provide information on ways to share data electronically through the HIE with other treating providers as part of a Survivorship Care Plan strategy. o To educate the audience on how the Survivorship Care Plan can be focused on information for the patient. o To give feedback on the new Commission on Cancer (CoC) review process for Survivorship Programs. Goals of the Webinar. By participating in the webinar, audience members will have a better understanding of the changing nature of Survivorship Care Plans and the larger Survivorship Program. The webinar will: o Assist cancer programs in using readily available HIE tools to more easily transmit information on a survivor’s care and treatment plan. o Make cancer centers more knowledgeable about the Commission on Cancer’s focus on Cancer Survivorship Programs through the insights of a CoC recently reviewed cancer center. As part of the discussion, cancer centers and providers will better understand how to shift the focus of Cancer Survivorship Plans to make it more patient-centric. Information that flows through the HIE can be used as the needed background for clinical treating providers, thus leaving the Survivorship Care Plan to focus on patient education and follow-up.
