Bad Parameter Sniffing - what is it, and what can you do about it?

Parameter sniffing has a bad name. But that is unjustified. Most of the time, parameter sniffing works to give us better performance. Except when it doesn't. Due to a bad interaction with plan caching, we can sometimes get bad parameter sniffing, resulting in hard to reproduce, erratic bad performance.
