NTSB: Scary Wire ERROR Caused Dali Crash/Key Bridge Collapse Baltimore

The National Transportation Safety Board (NTSB) has concluded that a single loose wire was a critical factor in the sequence of events that led to the container ship Dali striking and collapsing the Francis Scott Key Bridge in Baltimore. NTSB determined that wire 1, which was not properly seated in a circuit breaker terminal block 381, likely triggered the initial power outage on the vessel. The tragic incident resulted in the loss of six construction workers who were on the bridge at the time. Investigators found that a wire-label banding prevented the wire from being fully inserted into a terminal block, resulting in an inadequate connection.
