An L4-L5 or L5-S1 disc bulge on MRI does not automatically explain pain. A disc bulge describes shape, not symptoms. Pain usually depends on how the disc and spine behave during movement and load, not how the image looks. An MRI shows structure, but it cannot predict how your back will feel. This applies to people who have been told their MRI findings explain their pain or future risk. Disc bulges are common and often misunderstood because imaging language sounds severe. Many people without pain have the same L4-L5 disc bulge or L5-S1 disc bulge seen on MRI. The report does not show how your spine tolerates daily movement or whether a nerve is actually being irritated. An MRI report lists findings like bulge, protrusion, or degeneration based on appearance. It does not measure stability, load tolerance, or movement control. Pain usually develops when spinal motion, posture, or repeated stress irritates sensitive tissue or a nerve. Similar MRI findings can behave very differently depending on how the spine is used. What often helps is understanding movement patterns and reducing irritation. What often does not help is assuming the image alone defines the problem or rushing into invasive care without functional assessment.
