Ethik in der Pflege: Wenn Intensivmedizin an ihre Grenze kommt

Intensive care medicine can stabilize compartments. It can manipulate osmolarity, initiate dialysis, shift fluids, and artificially support circulation. But it cannot create meaning, restore a life story, or technically restore dignity. This is precisely where the ethical boundary of modern compartment medicine begins. Multiple organ failure is often a gray area: Circulation is functioning—with high doses of norepinephrine. The kidneys are functioning—during dialysis. The lungs are oxygenated—with ventilation. Physiologically stabilized, but the question arises: Is this still life or a prolonged dying process? A key principle is: Medical feasibility is not a moral obligation. Just because we can resuscitate, perform dialysis, or administer aggressive fluid therapy doesn't mean we always have to. Nursing acts as an ethical compass in this context. Nurses are at the bedside, directly experience the consequences of therapy, and bring a perspective of closeness. The most difficult phrase is not "We will continue to fight," but rather: "We must consider together what is in the patient's best interest."
