Is it possible to intercept a serious neurological complication before it compromises the mind and takes away the voice of entire families? In Rome, the institutional event "Beyond the Symptom, The Person" brought together clinicians, policymakers, and economists to shed light on the dramatic impact of hepatic encephalopathy. On this occasion, the campaign "Hepatic Encephalopathy - Can You See It?" was launched, promoted by Alfasigma in collaboration with the EPAC ETS association, with the aim of developing a structured national plan to combat this disease. What is hepatic encephalopathy and how is it recognized? Hepatic encephalopathy is a serious neurological disorder and a complication of liver cirrhosis, caused by the liver's inability to filter toxins, which reach the brain and compromise cognitive and motor functions. It manifests in various forms, from minimal (with subtle symptoms such as drowsiness or changes in the sleep-wake cycle) to full-blown forms that can lead to coma. Early identification of symptoms and triggers (infections, constipation, dehydration) is crucial to preventing the development of more advanced stages. The impact of this disease is severe, both for family stability and the sustainability of the National Health Service, resulting in over 10,000 hospitalizations per year and costs exceeding €200 million, fueled by a rehospitalization rate of up to 70%. To reverse this trend and address the hidden burden of an estimated 80,000 missed cirrhosis diagnoses, synergy between the various healthcare providers is essential. Acute management must be immediately integrated with community medicine: the strong clinical culture of hospital internists must be integrated with general practitioners, who are the first line of defense in the daily monitoring and management of fragile patients with metabolic cirrhosis. The new care paradigm, also supported by the Ministry of Health's guidelines and the hospital-territory delegation law, aims to ensure early diagnosis, consistent treatment pathways, and care that enhances the role of family caregivers and the third sector. Through local information campaigns focused on nutritional counseling and sharing sentinel symptoms, the aim is to reduce bureaucracy in access to care and implement the policy paper's priorities, transforming the guidelines into pragmatic regional measures to protect patients' dignity.
