If your hospital uses NEWS2, you are already aligned. 2️⃣ FLUIDS β€” Beyond the 30 mL/kg rule. The 30 mL/kg bolus is RETAINED but reframed as Phase 1 of a new 4-phase model: Resuscitation β†’ Reassessment β†’ Post-resuscitation Strategy β†’ Fluid Removal. Active fluid removal is entirely new. Albumin removed from resuscitation guidelines (ALBIOS trial). 3️⃣ ANTIMICROBIALS β€” Prolonged beta-lactam infusion upgraded from Suggest β†’ Recommend (Strong). First bolus as standard, then prolonged infusion for subsequent doses. Applies to piperacillin-tazobactam, meropenem, ceftazidime, and cefepime. De-escalation also upgraded to Strong Recommendation. 4️⃣ VASOPRESSORS β€” Norepinephrine downgraded from Recommend β†’ Suggest. Vasopressin and angiotensin II now valid first-line alternatives.
