This is rarely the case. Vehicles with a poor maintenance record or a history of abuse may exhibit shifting irregularities post-flush because the root-cause is no longer masked by contaminated fluid. The old fluid was so full of burnt clutch material, it actually increased the friction coefficient of the fluid. When replaced with new fluid, these worn clutches begin to present themselves. I have exchanged fluid on many transmissions with over 200,000 miles that likely never saw any kind of preventive maintenance.
