Following its transfer to private management in 1992 (Doc. ND-HQ-FAC-1992-0917), the site had been reassessed as "within manageable limits." A containment failure has since recurred at this location. ND-004 was sealed within the designated sub-level containment zone. However, it induced its own revival by luring and contaminating personnel within the facility. This is a confirmed case of active interference while in a sealed state. ■ ENTITY ASSESSMENT Compared with the initial containment failure of 1987, no significant change in the entity's influence or scale was observed. Despite 18 years of long-term sealing and monitoring, the entity has not weakened.
