Is it right to use FES – Functional Electrical Stimulation – after a stroke? In this video, I answer a question from one of our patients in the remote rehabilitation program. I decided to make it public because it's a common concern for many people: does FES really help regain walking ability, or does it risk fueling compensation and spasticity? First, I'll explain what functional electrical stimulation is: a device that sends electrical stimulation to muscles, such as the tibialis anterior, at the exact moment when the foot should dorsiflex during walking. On paper, it's an extraordinary technology. In practice, however, the situation is much more complex. In this video, I explain: • When FES might make sense and when caution is needed. • Why international guidelines include it among the recommended therapies. • The difference between recovery and compensation, and why rehabilitation today is truly flawed: "doing" is favored over the quality of movement, and then results are measured with scales that assess only the ability to perform a task, not how the task is performed. • Why neuroplasticity can be adaptive or maladaptive: you can achieve rapid independence through compensation, but in the long term, it becomes more difficult to achieve true restoration of function. • How neurocognitive rehabilitation completely shifts the paradigm, focusing on learning, perception, body representation, and the activation of the brain functions actually involved in movement. I also explain why, while I understand patients' desire to "try everything," it is crucial to choose therapies that are consistent with the way the nervous system works.
