This isn't an actual case, but a fictional one I created. However, because it's created by a clinician, I believe it has a certain degree of realism. I hope you'll find it useful. ■Case Introduction Ms. A is an 18-year-old girl who is constantly self-conscious about her appearance. She's constantly self-conscious about her appearance and her weight. She can't stop thinking about them. In fact, whenever she eats, she immediately steps on the scale. She gets extremely excited or upset when she gains weight. She gets excited or upset when her weight changes. She throws up because she doesn't want to gain weight. That's the kind of person she is. She eats even when she's telling herself, "I shouldn't eat, I shouldn't eat." She finally can't hold it in any longer and eats. But then she throws up. If she throws up everything, she can erase all the food she ate. She's obsessed with the allure of it. She's obsessed with the allure of "eating" and "vomiting." Why did Ms. A end up like this? Why does she continue to throw up even when her body is in ruins? I hope people will understand the pathology of eating disorders. Ms. A's family is one in which her mother is strict and her younger brother is extremely bright. Their father is either divorced or has already passed away. Ms. A's mother gets very angry with her, telling her, as the eldest daughter, "You need to get your act together," "Speak your mind," and "Do it properly." Her younger brother is just like her mother. She's strong-willed and always takes her mother's side, saying things like, "Big sister, do it properly." Her younger brother actually gets good grades and is quick to express his opinions. In contrast, Ms. A is not good at studying, has poor communication skills, is timid and has difficulty expressing her opinions, and has difficulty understanding her own feelings or the state of her body. She has a poor ability to self-monitor. She seems to have a poor ability to put into words and organize things that are messy, such as what's going on in society, what other people are like, what human relationships are like, and the state of her own mind, things that cannot be seen. That's why she's always fidgeting. Because he's fidgeting, his mother gets annoyed and says, "Are you trying to say it's my fault?", even though she doesn't mean it that way, and ends up becoming aggressive. His younger brother is also at the same adolescent stage, and he wants to monopolize his parents' love. He wants to monopolize his parents' love and their approval, and he doesn't really know who he is, or whether he has value or not. As the younger brother goes through adolescence feeling anxious, he wants his parents' approval. It's something he'll fight over or even fight to get, so he ends up pushing them away. It's like when a baby swallow desperately opens its mouth to be fed by its mother. It's like that. While the younger brother desperately seeks love, nourishment, and praise, Mr. A gives in to it. He says that under pressure, the parents end up giving only their younger brother nourishment. The mother wants him to be more responsible, but doesn't know any other way. With this background in mind, one day a classmate told her, "You've lost weight." On social media, they would comment, "You're so skinny," and "You look cute," and press "Like!" For A-san, who doesn't usually receive compliments, when someone tells her, "You've lost weight," it feels like a compliment, and her brain juices get really flowing. It surges with joy. She feels recognized and praised, and it makes her so happy she can't contain herself. When you're a teenager, especially when you're young, appearance is everything. Appearance is highly valued. If you're thin, you're more likely to get compliments and be told you're cute. After all, it puts you in a higher position than if you have great grades or great communication skills. If you're cute, it's sometimes okay to keep quiet, and everyone would ask her, "Teach me how to lose weight." But she would tell them, "No, I'm just not eating and trying to be patient," and she was incredibly happy. She was being humble, but incredibly happy. That's why you can't stop. You can't help but want to be told you've lost weight. You can't forget this brain juice, this excitement. So you hold back from eating. But you just can't hold back. In t...
