Excessive or repetitive yawning is a recognized—though rare—autonomic phenomenon associated with epileptic seizures, particularly those originating in the frontal or temporal lobes. It can act as a warning sign (aura), occur during the seizure itself, or appear as a post-ictal behavior. Yawning is primarily controlled by primitive brainstem and diencephalic structures, which regulate various involuntary functions like breathing, heart rate, and alertness. In patients with 22q11.2 Deletion Syndrome (22q11.2DS), the nervous system faces a higher risk of structural brain abnormalities (such as polymicrogyria), genetic generalized epilepsies, and endocrine issues. Seizures in 22q11.2DS are often driven by these complex neurobiological factors or provoked by severe hypocalcemia (low calcium due to hypoparathyroidism). Yawning in this context is generally categorized in a variety of ways: Ictal (During the seizure): Yawning occurs as a direct autonomic manifestation of the seizure itself, often originating in the temporal or frontal networks, or Peri-ictal (Preceding or following): The yawning acts as a premonitory sign or a "release phenomenon" as the nervous system recovers post-seizure.
