Occipital neuralgia, also known as Arnold's neuralgia, is a type of headache that affects the region innervated by the occipital nerve, which provides sensation to the back and sides of the scalp, from the nape of the neck to the top of the head and ears. Occipital neuralgia is caused by irritation, inflammation, or compression of the occipital nerve, and can result from several factors or clinical conditions, such as osteoarthritis of the upper cervical spine, degenerative changes in the spine, trauma to the occipital nerve, neck tumors, inflammatory diseases, and infections. Occipital neuralgia is described as a throbbing, stabbing, shock-like, needle-like pain that starts at the back and lower part of the head (nape of the neck) and rises to the top, possibly reaching near the eye and ear. The pain is very intense and constant, usually affecting one side of the head. The scalp becomes sore and sensitive to touch, making it difficult to wash and comb the hair, and also to rest the head on the pillow. The pain can be triggered or worsened by neck movement, especially rotation and extension, as well as by pressure on the nerve in the occipital region. The diagnosis of occipital neuralgia is made through clinical history and physical examination. Complementary tests, especially radiological examinations, may be requested in an attempt to clarify the cause of the problem. Other types of headaches can make the correct diagnosis of occipital neuralgia difficult. How is occipital neuralgia treated? Medications Initial treatment for occipital neuralgia is done with medication, and the doctor may use various classes of drugs, including anti-inflammatories, anticonvulsants, antidepressants, and muscle relaxants. Anesthetic Blocks Injections of local anesthetics, with or without corticosteroids, can be performed by the attending physician in the region of the occipital nerve. Botulinum toxin application can also be performed. Physical Measures Some patients may benefit from the application of warm or cold compresses and local massages. Neuromodulation Patients who do not improve with medication and infiltrations may benefit from neuromodulatory techniques of electrical stimulation, notably occipital nerve stimulation or spinal cord stimulation. Occipital nerve stimulation – A minimally invasive technique that consists of implanting a thin electrode under the skin, in contact with the occipital nerve and connected to the neurostimulator/generator, which, when switched on, sends mild electrical impulses that inhibit pain. Spinal cord stimulation – In this technique, an electrode is implanted in contact with the cervical spinal cord and connected to a neurostimulator/generator. When switched on, the electrical impulses sent to the spinal cord inhibit pain. Percutaneous procedures using radiofrequency on the occipital nerve and decompressive surgeries can also be used.
