Cold Knife Conization Procedure Explained

Cervical cancer is the 4th most common cancer in the world with 660,00 new cases every year and 350,000 deaths. Cervical cancer can be cured if it is caught early. The pap test is one of the best screening tests available. We are able to detect HPV related changes early with the routine pap test. Human papillomavirus is a common sexually transmitted infection. HPV rarely causes symptoms. In most young, healthy patient’ their immune system will clear the HPV virus. However, some patients will have a persistent HPV infection that is not cleared and the persistent HPV infection can lead to changes on your cervix which are later picked up on the pap test. Smokers, women with weakened immune systems have a harder time clearing the HPV infection. In the US, we commonly will check an HPV test at the time of a pap smear if you are over 30. Your doctor may also offer your primary HPV screening after the age 25. Please see a few of my other videos for more information on the pap test and cervical cancer screening. If you have an abnormal pap test, the next step is to have an office colposcopy with biopsy to evaluate your cervix. If your biopsy comes back with CIN 1 or mild dysplasia then we will often repeat your pap test in 1 year. If your biopsy comes back abnormal with CIN 2-3 or moderate to high grade cervical dysplasia then your health care provider may advise you to have a cold knife conization of the cervix.
