Some of the dolls are in plain sight, but many others are hidden inside rocks and specific enemies, which are only revealed when you walk into them - pretty much impossible to find without repeated play, especially when touching the wrong thing kills you! As for the letters, collecting one in rounds 1, 2 and 3 of each area results in an extra life being hidden somewhere in round 4.
