Remember that not every movement can be performed for every disease.* DIAGNOSIS AND TREATMENT OF HIP PAIN The hip joint is one of the largest and most mobile joints in the body. It has a strong structure that is resistant to constant intense movements and wear and tear. Although it experiences relatively fewer problems compared to other large joints, severe pain and movement disorders can develop. Hip pain can originate from the joint, the soft tissue around the joint, the bone, and pain radiating from other organs. The most common causes of hip pain: 1- Osteoarthritis: This is one of the most common causes. It usually develops due to congenital disorders, inflammatory rheumatism, trauma, and fractures. 2- Inflammatory rheumatic diseases: (such as rheumatoid arthritis, ankylosing spondylitis) 3- Hip fractures: Develop in the elderly, especially as a result of increased fragility due to osteoporosis. 4- Bursitis: Inflammation of the bursae around the hip. Most often develops with inflammation of the bursa on the bony prominence on the outer side of the hip. 5. Muscle and tendon strains: Can occur as a result of overuse or trauma. Injury can be in the form of a complete or partial tear and can cause hip pain. 6- Avascular necrosis: Impaired blood flow to the hip bone leads to nutritional problems and edema and destruction of the bone. This causes severe pain. 7- Cancers: Can occur from the hip itself or from the spread of other cancers, which we call metastasis. 8- Osteoporosis 9- Frozen hip joint 10- Nerve damage 11- Infections 12- Hip impingement syndrome (Femoroacetabular impingement syndrome): Enlarged hip head 13- Congenital disorders: Hip dislocation, etc. 14- Referred pain: Can be referred from lumbar disc herniations and osteoarthritis, pelvic organs, kidney, appendicitis, or major artery. 15- Piriformis syndrome What are the symptoms of hip pain? Pain and associated limited movement are common. The pain is located on the front, side, back, or medial side of the joint. It can occur in one or both hips, during movement or at rest. It can lead to gait disturbances over time. How is hip pain diagnosed? Based on the patient's history and clinical examination, laboratory tests and imaging are requested. Blood tests are particularly important in cases of fever, night pain, and weight loss. Imaging techniques are crucial in cases of trauma, congenital diseases, and poorly localized pain. Radiological examinations include plain radiography, computed tomography (CT), bone density measurement, and magnetic resonance imaging (MRI). Treatment of Hip Pain 1. Medications: Medications can be used to reduce pain in hip pain. For this purpose, analgesics/anti-rheumatic drugs, muscle relaxants, and opioids (depending on the situation) can be used. In the presence of osteoporosis, calcium, vitamin D, and osteoporosis medications should be added to the treatment. 2. Rest: Rest can be applied to reduce weight-bearing. In cases of hip pain, except in cases of trauma, short periods of rest are recommended rather than direct bed rest. 3. Physical Therapy: The combined use of physical therapy tools in hip pain is recommended as it has been a successful treatment method for many years. It can control pain in the hip area by reducing muscle spasms, decreasing edema, and increasing blood flow. Superficial and deep heat, cold, electrotherapy, high-intensity laser, radiofrequency treatments, etc. can be used. 4. Exercise: Both hip-specific and aerobic exercises are recommended. For hip-specific exercises, strengthening the muscles that provide hip stability, stretching the painful leg, and regularly performing deep sensation-enhancing exercises are recommended. Aerobic exercises should also be added for both pain and weight control. 5. Complementary medicine methods: Ozone therapy, mesotherapy, neural therapy, and kinesiological taping can be used as complementary medicine methods to control hip pain. 6. PRP: Reconstructive and reparative treatment can be performed by injecting platelet-rich plasma into the hip. Detailed information can be found in the PRP section. 7. Intrahip fluid supplementation: This is performed by injecting a fluid similar to joint fluid into the hip. 8. Weight control: Since every pound lost due to weight bearing reduces the load on the hip, weight loss is recommended. 9. In patients with severely worn hip joints and pain that cannot be relieved otherwise, surgery may be necessary.
