Begin by thoroughly washing your hands with soap and drying them. Then perform hygienic hand disinfection. Measuring the stoma length: To prevent pressure sores, skin irritation, and leaks, the stoma length of the button should be measured regularly to determine the required button length. Indications that you should measure the stoma length include: significant weight gain or loss, a very tight-fitting tube, leaks at the stoma, a very loose-fitting tube, or skin irritation around the stoma. 1. To measure the stoma length and to change the button, first remove the old button. To do this, connect the Luer syringe to the inflation valve, which is often labeled "Inflation" or "BAL." Now withdraw the fluid from the balloon and carefully pull the old button out of the stomach. 2. To measure the stoma length, apply water-soluble lubricant or water to the tip of the length gauge to make it lubricated. Then carefully insert it into the stoma. 3. Inflate the length gauge balloon with the required amount of fluid using a Luer syringe. 4. Pull the length gauge back until you feel resistance; this indicates that the inflated balloon is resting against the stomach wall. 5. Now lower the plastic disc until it reaches the skin. The patient should be lying on their back for this. 6. Read the stoma length above the retention plate and add 4-5 mm to ensure the correct stoma length and fit for each position. Record the measurement. 7. Finally, remove all the water from the length gauge balloon using a Luer syringe and slowly and carefully withdraw it from the stoma. Procedure for changing the button: 1. First, check the button balloon for leaks. To do this, fill the balloon with the recommended amount of fluid. The required amount is often indicated on the inflation limb or can be found in the user manual. 2. Draw the fluid into the Luer syringe and connect the syringe to the inflation valve. Apply the fluid into the balloon. The balloon should be symmetrical. Check the balloon for leaks. 3. You can then remove the fluid using the Luer syringe. 4. To make insertion of the button into the stoma easier, apply water-soluble lubricant to the tip of the new button. Using a guide stick can also facilitate placement. 5. Insert the button into the stoma until it lies flat against the skin. 6. Secure the button with one hand to prevent it from slipping out and inflate the balloon with the specified amount of fluid. 7. If necessary, remove any remaining lubricant from the skin. 8. You can now start feeding or take a feeding break. Important note: Do not inflate the balloon with air, as air escapes quickly and the button or gastrostomy tube may shift position. The balloon should also not be overinflated, as it can easily burst. It is recommended to always have a spare replacement tube at home in case of emergency, as the balloon of the replacement tube can burst unexpectedly, and the stoma will close within a few hours if the button or gastrostomy tube is not in place.
