In this Video Lesson from the Anaesthesia Unravelled course, we’ll be talking about the “Checklist” of anaesthesia. The checklist is of utmost importance in any activity that requires organisation, and anaesthesia is certainly one of them! The checklist is strongly related to the quality of the procedure and might spare us from some complications that can be met during anaesthesia. Therefore, we will be dividing the method into: 1- Patient Checklist: We need to confirm who the patient is, what procedure is to be performed and cross-check the patient's record with the information on anamnesis, physical and complementary exams. A very important detail is the consent of the owner regarding the procedure. Remember that anaesthesia is not devoid of risks and the guardian must be aware of the entire process. 2- Checklist of medical gases: Regardless of the anaesthetic modality, be it inhalation, dissociative, TIVA or locoregional, the supply of oxygen is essential to maintain the animal's PaO2 at acceptable levels. Thus, we must make sure that there is enough O2 for the anaesthetic procedure and also compressed air, to be used in mechanical ventilation or in a possible mixture of gases. We should go through everything, from the cylinder gas outlet to the network’s connection to the surgery room. 3- Anaesthesia Machine Checklist: Carrying on, we test the anaesthesia machine. Even if you are not using inhalation anaesthesia at a given time, you must still make sure that it works correctly to provide O2 and / or use mechanical ventilation on the patient. In this case, we should test the flowmeters, make sure there is enough inhalation anaesthetic and soda lime for the procedure and perform a leak test in the circuit, even when using a non-rebreathing one. 4- Medication checklist and access: We must make sure that we have all the necessary drugs for the procedure available, especially emergency ones. All must be much organised, in a cart or briefcase. We must obtain the venous access and prepare the material for intubation. Again, even if the animal is not undergoing inhalation anaesthesia, it may need mechanical ventilation during the procedure. Prevention is paramount. 5- Equipment Checklist: We should verify infusion pumps and monitors. For infusion pumps, check the desired rate and flow and programme everything beforehand. If the anaesthesia is a TIVA, the pumps for IV fluids and medications must be different; preferably, an infusion pump for each medication. As for monitoring, we must go through all the equipment, see if the devices are working correctly and position all sensors and clamps so that we can connect them to the patient in the shortest possible time. The choice of parameters assessed will depend on the animal's physiological state and type of procedure. Common sense is essential! 6- Checkout: Organisation is also required at the end of the procedure! After the conclusion of the anaesthesia, we must make sure that the gas flow has been closed, extubate the animal only when it has an active cough reflex, check the postoperative analgesic prescription and explain everything to the owner. We must still clean the entire equipment, organise the monitoring cables and tidy up as best as possible. Want to know more?
