Minoxidil doesn't work for some people. In this video, Dr. Paulo explains why not all patients improve with Minoxidil treatment. There are numerous studies that prove the overall effectiveness of Minoxidil. And when we talk about this effectiveness, we are referring specifically to the treatment of Androgenetic Alopecia (baldness). These studies consist of evaluating two groups of patients, where one group will use Minoxidil to treat hair loss, and the other group (control group) will use a placebo (a solution without minoxidil). In these studies, the group that uses minoxidil consistently shows greater improvement than the placebo group. But if the medication is effective, why do many patients not notice an improvement with the use of Minoxidil? One possibility is incorrect diagnosis. The effectiveness of minoxidil has been proven more clearly only for androgenetic alopecia. Another problem is the inappropriate use of the product or use for a short period of time (less than 6 months). Many patients improve, but an accurate assessment is not performed with standardized photos. Now let's consider a patient who really has Androgenetic Alopecia (baldness), has undergone the treatment appropriately, with standardized clinical photos, and has not really shown any improvement. What could be happening? The reality is that although several studies prove the effectiveness of Minoxidil, these studies evaluate groups of patients, and within the group there are variations from one patient to another, with some improving more, others less, and some not having a significant result. In other words, Minoxidil is not effective for everyone. When we carefully evaluate patients, we realize that about 30 to 40 percent of them do not have a satisfactory response to the use of Minoxidil (oral or topical). And why do some patients not respond to the medication? This is not an exceptional situation in medicine; in fact, it is normally like this. Most treatments are not 100% effective, and Minoxidil is no different. Particularly in the case of Minoxidil, there is a theory about the activity of an enzyme called Sulfotransferase. This enzyme is responsible for transforming Minoxidil into Minoxidil Sulfate, which is the substance that actually stimulates hair production. In some people, this enzyme has a higher activity, which would promote a greater conversion of Minoxidil and consequently better results. However, in some people this enzyme has a lower activity, which would lead to less effectiveness. Although this theory is very interesting, the data that supports it still comes from small studies. Therefore, we still need more data to state unquestionably whether this enzymatic activity is decisive or not in obtaining results with Minoxidil.
