How to replace throttle/butterfly shaft seals to fix lean idle, idle hang and poor idle in general

In most cases this is caused by an air leak on the inlet side of the engine, which causes the mixture to lean out, the leak can be from old cracked carb rubbers or in many cases failing throttle/butterfly shaft seals that are not easy to replace or even find parts for.
