Prof Joseph Neu on the Controversies in the Nutrition of ELBW babies: Practice aggressive nutrition in ELBW babies with 3 gm/kg of lipids and 3 – 4 gm/kg of proteins from day1 of life and grade up enteral feeds at least 30 ml feeds/kg/day! Essential fatty acid deficiency occurs in 80% of preterms after 1 week of lipid free nutrition! Is monitoring triglycerides necessary? Don’t shoot the messenger! Hyperlipidemia on first few days in aggressive nutrition is not the cause of your problem, might even be the effect of your problem! Fortify the Donor milk! Donor milk is preferred over formula if MOM is not available in ELBW Babies and baibes below 32 weeks gestation. However, it should be Fortified to meet the nutritional demands of the ELBW baby Human milk based fortifiers are not necessarily better than conventional fortifiers? There is not enough evidence to say that cow’s milk-based fortifiers are inferior to human milk based fortifiers! Watch out for the results of adequately powered RCTs in this regard (eg: Studies should at least have 1500 babies to evaluate NEC) Why precision nutrition in neonates? One size doesn’t fit all! If we see growth faltering, it might already be too late! Personalized nutrition is the way to go! Under research: Multi-omics approach, use of clinical and microbiome data and Artificial Intelligence might be of use in optimizing nutritional requirements in the future!!!
