She also had grade 3 obesity. Following evaluation, RNY gastric bypass surgery was planned instead of sleeve gastrectomy. This is because, in cases of severe reflux, sleeve gastrectomy can worsen symptoms in some patients, while RNY gastric bypass surgery provides both weight loss and stands out as a powerful surgical option for reflux treatment. Our patient had been suffering from long-standing gastric reflux and bile reflux, causing significant symptoms such as nighttime discomfort, regurgitation, heartburn, and the need for continuous acid-suppressive medications. In addition, she was diagnosed with class III obesity. After comprehensive evaluation, Roux-en-Y gastric bypass was selected instead of sleeve gastrectomy. In patients with severe reflux, sleeve gastrectomy may worsen symptoms, whereas gastric bypass is considered an effective surgical option for both weight loss and reflux control. This video was recorded 48 hours after surgery, just before discharge. The patient reports noticeable improvement in reflux symptoms in the early postoperative period. Bei ausgeprägtem Reflux kann der Schlauchmagen die Symptome verschlechtern, während der Magenbypass sowohl eine effektive Gewichtsreduktion als auch eine Verbesserung der Refluxsymptomatik ermöglichen kann.
