The lunate is one of the 8 small bones in the wrist. The lunate bone is located in the middle of the wrist and, together with the two small bones on either side (scaphoid and triquetrum), forms the wrist joint. It also articulates with the forearm bones (radius and ulna) at the top. For reasons not fully understood, impaired circulation in this bone leads to a loss of its normal structure. This loss of the lunate bone's normal structure and change in shape can cause pain, limited movement, and osteoarthritis in the wrist. It most commonly occurs between the ages of 20 and 40. Very rarely, it can affect both wrists simultaneously. Skeletal variations (such as abnormal length discrepancies in the forearm bones (ulna, radius)) and repetitive wrist trauma are contributing factors. The disease manifests as pain in the middle of the wrist. Increased pain upon raising the middle finger is characteristic. The pain in the wrist joint progresses progressively. While there may be no direct radiological findings in the early stages, over time, deformities, collapse, sclerotic areas, and fragmentation of the lunate bone may appear. MRI and CT scans are useful in diagnosis. Especially if suspected in the early stages, MRI imaging is crucial for early diagnosis. Diagnosis is difficult in the early stages. It can often be overlooked if one is not careful. Pain and symptoms increase after a period of time following the onset of the disease. Often, diagnosis is only made a long time after the disease begins. Generally, the disease develops slowly and progressively. It is divided into 4 stages. In the first stage, radiological examination is usually normal. MRI helps in diagnosis. The lunate gland begins to lose its blood supply; small fractures may occur. In the second stage, the shape of the lunate gland begins to deform. In the third stage, the lunate gland has collapsed. The lunate gland has completely collapsed, and osteoarthritis has begun in the wrist joint. The stage of the disease, the presence or absence of a length discrepancy between the forearm bones, and the patient's age all influence the treatment decision. In very early stages, a period of rest with a splint is sufficient. In advanced stages (stages 2-3), correcting the existing length discrepancy between the forearm bones is beneficial. This not only solves mechanical problems but also improves bone circulation by increasing blood flow. If there is no difference in length, vascularized bone grafts should be considered as a treatment option. In more advanced stages, fusing small bones together can change the direction of load transfer. If the wrist joint is osteoarthritis, wrist fusion may be planned. In recent years, treatment methods using camera systems have become popular. Deciding on the treatment of the disease can sometimes be difficult.
