In this video, Dr. Paulo discusses the use of antibiotics for acne treatment, in which situations they can help, and the best strategies for their use. Acne is an extremely common problem, especially during adolescence. Approximately 80% of teenagers suffer from acne. In fact, there are several treatments we can use for acne. From applying soaps to reduce oiliness in the skin (in simpler cases), to the use of isotretinoin and antibiotics in more intense and specific cases. There are also cases where the patient develops acne in response to another factor, such as the use of certain medications like corticosteroids, or even due to a hormonal problem, such as polycystic ovary syndrome. Therefore, it is important to understand that the diagnosis and monitoring of each patient need to be individualized, as acne can have different degrees and causes from person to person, and each person responds differently to treatment. So, speaking specifically about antibiotic treatment, in what situations can they help? For patients suffering from a more inflamed lesion pattern, with many red bumps and pustules, antibiotics can be very helpful. Even patients with larger lesions and deeper nodules can benefit from antibiotics, but generally, in these cases, this alone will not be enough, and treatment will need to be supplemented with Isotretinoin (Roaccutane). In simpler cases, where the person only has blackheads with little inflammation, the patient tends not to improve with antibiotics. And how is oral antibiotic treatment for acne done? The patient needs to complete a course of treatment, usually from 30 to 90 days. Several antibiotics can be used, such as Azithromycin, Tetracycline, Doxycycline, and perhaps the most commonly used currently, Limecycline. So, for example, in the case of Limecycline, the dose can vary from 150 to 300 mg per day. At the beginning of treatment, we may start with a higher dose, around 300 mg. Sometimes, after 30 or 45 days of treatment, this dose may be reduced to 150 mg per day until the end of treatment, for example. Of course, the type of antibiotic, the dose, the monitoring, or even whether the antibiotic is the most appropriate treatment for your case, depend on the evaluation and guidance of your dermatologist. Do not self-medicate. A very important fact that needs to be understood is that antibiotics cannot be maintained permanently. And this is where we have the biggest problem with this type of treatment, because, in many cases, despite the improvement, when we finish the treatment cycle, there is a tendency for acne lesions to recur. Unfortunately, in general, the use of antibiotics alone does not have good efficacy in the medium and long term. Of course, some patients experience more persistent results at the end of the treatment cycle, but some degree of relapse is more common. Therefore, it is important for the doctor to develop a treatment strategy to achieve more lasting results. For this strategy to be successful, an accurate diagnosis is crucial, and the particularities of each patient must be taken into consideration.
