You Matter - End of Life Care in the Community
You Matter is a 17-minute film focussed on care in the last weeks of life in a community healthcare setting. A suite of educational materials have been created to support the film, including slide sets, handouts, notes for facilitators and evaluation tools. These can be used to deliver end of life care education for community settings, and includes specific modules on the following aspects of care: • Preparing for a death in the community; including recognition of deterioration, prognostication, bereavement, cultural and spiritual issues • Effective communication • Making a plan of care for the last days of life • Physical care in the last days of life • Advanced Care Planning: discussions and decisions To download and read more about the film and educational materials, or to get in touch with the UCLPartners End of Life Care team, go to www.uclpartners.com/case-study-eolc/ The You Matter film was developed by East London NHS Foundation Trust and is used with their permission.
