In this video, I discuss the role of liothyronine (T3) as a potentiation strategy in treatment-resistant depression, bringing a practical, evidence-based perspective—which, in my view, is still underutilized in clinical psychiatry. Based on classic data such as STAR*D (step 3), it is clear that T3 presents a particularly interesting combination: efficacy comparable to other strategies and, most importantly, better tolerability. In practice, this translates into a lower dropout rate due to adverse effects when compared to lithium, something that directly impacts clinical outcomes. Furthermore, I discuss a point that often completely changes the patient's experience: the speed of response. T3 can produce initial improvement within a few days, making it especially useful in scenarios of refractoriness or significant functional impairment. Throughout the lesson, I organize the use of T3 in a practical way: when to prescribe it, how to start (including initial dose and titration), which tests to order, and how to monitor it safely. I also address patient selection criteria, cardiovascular risks, and absolute contraindications, which need to be considered before prescribing. More than reviewing the literature, the goal here is to structure clinical reasoning: when T3 should be included in the therapeutic algorithm and why, in many cases, it may be a more rational choice than more frequently used strategies.
