Rotator Cuff Tear Exam (REAL Patient Demo)

Did this 83-year-old man tear his rotator cuff after a fall on the ice? In this video, I evaluate Keith, an 83-year-old gentleman who suffered a significant shoulder injury after slipping on ice and falling approximately five months ago. Despite severe pain, bruising down the arm, and ongoing weakness, his initial X-rays showed no fracture. During this orthopedic shoulder evaluation, we examine: ✅ Shoulder range of motion ✅ Strength testing ✅ Empty Can Test (Supraspinatus) ✅ Belly Press Test (Subscapularis) ✅ Bear Hug Test (Subscapularis) ✅ Lift-Off Test (Subscapularis) ✅ Assessment of possible biceps tendon injury ✅ Differentiating rotator cuff tears from cervical spine-related pain Based on the findings, we discuss why a massive rotator cuff tear involving the supraspinatus, infraspinatus, and subscapularis, along with a possible long head biceps tendon tear, is highly suspected. We also discuss why some patients with large rotator cuff tears and underlying arthritis may ultimately require a reverse total shoulder replacement.
