The Teamwork for Transformational Care Affinity Group meeting featured a presentation by Yelena Korpacheva, Danielle Lagana Treat, and An Peterson on the new Event Parameters Project, which launches June 1st across Stanford Health Care's Midas system. The project aims to standardize and simplify the contributing factors section in the SAFE (Stanford Alerts from Events) reporting system by reducing over 700 existing options to fewer than 50 standardized contributing factors organized into categories including human level, patient level, team, task, tools and technology, work environment, leadership, culture, policy and procedure, data and information, and external factors. The presenters explained that near-miss events occur far more frequently than adverse events but are less reported, making them crucial for identifying systemic flaws before patient harm occurs. The new standardized list will help improve data collection, identify trends for proactive prevention rather than reactive responses, and support continuous quality improvement efforts across the organization.
