Have you ever encountered a patient with frankly decompensated heart failure, despite unremarkable structural echocardiographic findings—preserved ejection fraction, normal chamber dimensions, and ventricular thickness without significant alterations? Did you consider constrictive pericarditis? And, more than that, did you feel confident interpreting findings such as annulus reversus, septal bounce, and septal shift? In this episode, we will unravel constrictive pericarditis: from clinical diagnosis to the interpretation of multimodal imaging, always based on an understanding of the pathophysiology of the disease—a central element in recognizing its findings and managing the case appropriately. We will also discuss individualized treatment, whether clinical, invasive, or combined. Throughout the conversation, we will show that, in constrictive pericarditis, nothing is absolute: the pericardium is not always thickened, nor is it always calcified, and the constriction is not always irreversible. Although relatively rare, constrictive pericarditis has a significant prognostic impact when not recognized and managed correctly. This is especially relevant because heart failure secondary to constriction can be reversible—provided that diagnosis and treatment are made at the right time.
