(CAS) Dementia represents a significant public health challenge related to population aging, being one of the main causes of disability and dependency. According to WHO data, in 2015 Alzheimer's disease and other dementias affected 47 million people worldwide, and this number is estimated to double in the next 20 years. Disease-centered care often leads to referrals to hospitals in end-of-life situations, increasing the risk of artificial nutrition, intensive treatments, and costs, while also decreasing family satisfaction and underutilizing palliative care resources. A different approach, with a person-centered care model that seeks maximum well-being and comfort, along with adequate support and information for the family, is possible through advanced care planning. This involves assessing the person's current needs and taking into account their values and preferences. This webinar series presents comprehensive care for people with advanced dementia from various perspectives. (EUS) Dementzia osasun publikoaren erronka garrantzitsua da biztanleriaren zahartzeari dagokionez, eta desgaitasunaren eta mendekotasunaren arrazoi nagusietako bat da. OMEren datuen arabera, Alzheimerren eta beste dementzia batzuen gaixotasunak 47 milioi pertsonari eragiten zien 2015ean, eta kalkuluen arabera, hurrengo 20 urteetan bikoiztu egin daiteke. Gaixotasunean oinarritutako arretaren ondorioz, normalean ospitaleetara bideratzen dira bizitza-amaierako egoeretan, nutrizio artifiziala erabiltzeko arriskua areagotuz, treatmendu intentsiboak eginez, kostuak areagotuz, familien gogobetetasuna murriztuz eta baliabide aringarrien erabilera gutxietsiz. Pertsona ardatz duen arreta-eredu bati bestelako begirada bat ematea, ahalik eta ongizate eta erosotasun handiena bilatuz, familiari laguntza eta informazio egokia emanez, possible da zainketen plangintza aurreratu baten bidez, pertsonaren egungo prizek ebaluatuz eta haren balioak eta lehentasunak kontuan hartuz. Webinar series honetan hainbat ikuspuntutatik azaltzen da dementzia aurreratua duten pertsonentzako arreta integrala.
