What is Gleason 7 and why is it so dangerous for you? In addition to the risks of prostate biopsies causing cancer and metastasis, there are the dangers of overdiagnosing cancer. The Gleason classification was developed to stratify the histopathological findings of biopsies. It is based on cellular differentiation and undifferentiation, as well as the amount of intercellular stroma and other important variables. However, in real-world practice, the differences are not absolute and the boundaries are not clear, making the classification arbitrary and prone to errors. Previously, the most frequent result for prostate biopsies was Gleason 3 + 3, or 6, which we have insisted for years does not correspond to cancer. This assertion is based on prostate biopsy results and the autopsy results of patients who died from septicemia caused by prostate biopsies in which cancer was not found when these values ​​were obtained. We describe with images how the Gleason classification is attempted and show what the needle is like and how the cylinders are formed from the women, as well as the difficulty of obtaining significant samples. We also show how inflammation affects histopathological results. We demonstrated how, as the number of samples increases, so does the possibility of a false cancer diagnosis, since the inflammatory reaction increases proportionally to the number of perforations. Therefore, inflamed tissue can easily be misdiagnosed as 4+3, or higher. For this reason, the most overdiagnosed cancer patients—understanding overdiagnosis as a misdiagnosis of cancer—are those on the Gleason scale 3+4 and 4+3. This will result in unnecessary worries and very likely unnecessary cancer treatments with serious side effects, and/or the loss of their prostate and/or testicles.
