In this video, we will explain how craniosynostosis is diagnosed. In the vast majority of cases, craniosynostosis is initially suspected by the parents or the pediatrician who is monitoring the baby. What is noticeable is an alteration in the shape of the skull... Instead of having the characteristic rounded shape, the baby's head may become elongated, flattened, or "crooked!" Although classically the diagnosis of craniosynostosis is confirmed with the gold standard - cranial tomography with 3D reconstruction - other exams are gaining prominence in the diagnosis of this disease. Generally, the first exam requested, even by the pediatrician when this disease is suspected, is a simple skull radiograph (the famous X-ray). However, radiographs, as well as CT scans, are performed using radiation, and there is enormous concern about the exposure of children to radiation, especially very young babies. This is because we already know that excessive radiation exposure is a risk factor for a number of tumors (cancer). Cranial ultrasound (USG), more specifically cranial suture USG, has been shown in several studies to be an excellent initial exam to rule out craniosynostosis, but it is underutilized in this context and unknown to many professionals who do not work with children. It offers excellent imaging. Ultrasound (USG) is a tool to visualize surface structures with the potential to confirm or rule out fusion of the cranial sutures (craniosynostosis), avoiding exposure to ionizing radiation in infants. The normal "gap" of an open suture or obliteration in craniosynostosis can be clearly demonstrated with ultrasound in children under 12 months of age in approximately 99% of cases. Two considerations are important: 1- Ultrasound classifies the suture as: open, closed, or indeterminate. Although an open suture practically excludes the diagnosis of craniosynostosis, if the suture is closed or cannot be determined, this baby should be immediately referred to a pediatric neurosurgeon for further appropriate evaluation. 2- Ultrasound is an examination that depends heavily on the technical skill and training of the physician performing it. Therefore, a physician without experience in this examination may not be able to visualize the sutures and provide an accurate diagnosis.
