Collecting a routine urine specimen

Identifies self by name. Identifies resident by name. Washes hands. Explains procedure to resident, speaking clearly, slowly, and directly. Maintains face-to-face contact whenever possible. Provides privacy. Puts on gloves. Fits hat to toilet or commode, or offers bedpan or urinal. Asks resident to void into hat, urinal, or bedpan. Asks resident not to put toilet paper in with the sample. Provides a plastic bag to discard toilet paper separately. Provides resident with supplies and asks resident to clean hands with wipe when finished. Removes and discards gloves. Washes hands. Places call light within reach and leaves room until resident calls. When called, returns and puts on clean gloves. Provides perineal care if help is needed. Takes bedpan, urinal, or hat to the bathroom. Pours urine into specimen container, making it at least half full. Covers container with lid. Wipes off the outside with a paper towel. Places the container in clean specimen bag. Discards extra urine. Rinses hat, urinal, or bedpan and flushes toilet. Places equipment in proper area for cleaning or cleans it according to policy. Removes and discards gloves. Washes hands. Returns bed to lowest position. Places call light within resident’s reach. Takes specimen and lab slip to proper area. Documents procedure.
