Sleep is a Vital Sign with Special Guest Dr. Maria Guzman

In this Lunch & Learn, Dr. Kara Pepper is joined by Dr. Maria Guzman, a board-certified sleep specialist and founder of Dream Life Medicine and Wellness, for a practical conversation about sleep, fatigue, insomnia, nervous system regulation, and why sleep problems are often more complex than “just relax.” Dr. Guzman explains how sleep is shaped by hormones, circadian rhythm, sleep pressure, nervous system activation, inflammation, chronic illness, and life stage changes, especially perimenopause and menopause. She also discusses how sleep issues can show up differently in women, people with chronic illness, people with dysautonomia, and those with ADHD or neurodivergent brains. This conversation is especially helpful for anyone dealing with insomnia, fragmented sleep, 2–3 AM awakenings, fatigue, POTS, MCAS, EDS, chronic illness, burnout, or waking up unrefreshed even after a full night in bed. In This Episode Dr. Pepper and Dr. Guzman discuss: Why sleep is a vital sign, not a luxury The difference between sleepiness, tiredness, and fatigue How cortisol, adenosine, caffeine, and melatonin affect sleep Why morning light, movement, and meal timing matter How chronic stress and hyperarousal can keep the brain awake Why inflammation, autoimmune disease, pain, trauma, and dysautonomia can disrupt sleep How poor sleep and inflammation can create a vicious cycle Why “sleep maxing” culture can create more anxiety around sleep How sleep changes with age, perimenopause, and menopause Why women may be underdiagnosed with sleep apnea What 1–3 AM awakenings may mean How alcohol can disrupt sleep later in the night When to consider a sleep study Why home sleep studies may not tell the whole story Sleep challenges in ADHD and neurodivergent brains Nervous system regulation tools, including breath, self-compassion, and EFT tapping How at-home sleep testing technology is evolving Key Takeaways: Sleep is controlled by more than bedtime habits. Hormones, nervous system state, inflammation, chronic illness, medications, circadian rhythm, and life stage all affect how well someone sleeps. Fatigue and sleepiness are not the same thing. True sleepiness means you may nod off if given the chance. Fatigue can be mental, emotional, physical, metabolic, or related to chronic illness. Waking up around 2 or 3 AM can happen because REM sleep increases later in the night. But frequent awakenings, gasping, heart racing, night sweats, or waking unrefreshed may be worth discussing with a physician. For women, sleep apnea does not always look like the classic stereotype of a loud-snoring older man. Women may present with fragmented sleep, frequent awakenings, fatigue, or insomnia-like symptoms. The goal is not to force sleep. Sleep is not fully voluntary. Instead, Dr. Guzman recommends creating the conditions that allow sleepiness to arrive. Practical Sleep Tips Mentioned: Get morning light early in the day Move your body, especially earlier in the day when possible Keep wake-up time consistent Avoid late-night meals when possible Don’t go to bed unless you’re sleepy Put your phone away before bed If you can’t sleep after about 20 minutes, get out of bed and do something calming Avoid beating yourself up during nighttime awakenings Use calming tools like breathing, a hand on the chest, quiet reassurance, or tapping Talk with a physician if sleep disruption is frequent, worsening, or affecting daytime function When to Talk to a Doctor About Sleep Consider talking with your physician or a sleep specialist if you experience excessive daytime sleepiness, waking unrefreshed, frequent nighttime awakenings, snoring or gasping, non-refreshing naps, sleep inertia, chronic insomnia, or fatigue that interferes with daily life.
