InsightHive 2026 | The Integration Gap: Connecting Hospitals, Primary Care and Community Systems

Across the NHS and wider health system, the ambition of a unified patient view is widely shared but still difficult to achieve in practice. Hospitals, primary care and community services continue to operate across fragmented digital landscapes, each with different systems, data standards, and operational pressures; creating gaps in visibility, duplication of effort, and challenges in how information moves between care settings. As organisations look to improve integration at scale, the challenge is no longer just about connecting systems. It also involves how workflows align across organisations, how data is governed and shared, and how operational processes adapt across multiple systems and providers. This session brings together perspectives from the EPR Network community: interoperability strategy, integration delivery, clinical practice and operational workflow design to explore what is happening on the ground — and what is needed to move closer to a joined-up patient view in practice.
