There are a number of diseases that cause such symptoms, and patients, mostly men, often seek help with these problems. First, what diseases can cause such symptoms? The most common are fungal infections of the skin between the toes. These can be dermatophytes, candidal fungi, or yeast-like fungi. The diagnosis is generally made and confirmed by scraping the skin and interdigital spaces, or by PCR diagnostics of the erosion itself, when the laboratory determines which specific fungus caused the inflammatory process and the formation of cracks. Third, there are bacterial cracks between the toes. They mainly occur after some kind of trauma to the skin in these areas, such as increased friction, diaper rash, excessive sweating, or the toes, when microtrauma occurs, where bacteria enter, causing inflammation, and, consequently, cracking. Third, there are eczema and other chronic skin diseases that can localize in the interdigital spaces, including oozing and peeling. Skin cracks and edema may develop. All three of these groups of diseases have one thing in common: this is the main cause of skin cracks between the toes: they become infiltrated and swollen, and their stretchability is worse. Since the toes are quite active, the areas between the toes can become stretched. As a result of these mechanical, physical effects of skin stretching, tears in the epidermis and sometimes in the deeper layers of the skin occur, resulting in cracks. How is the diagnosis made? First, to rule out a fungal infection, as I said, a scraping is taken for fungi. If erosion and the crack itself are present, a fresh scraping is taken for DNA testing of yeast-like fungi, such as Candida albicans and other types. If we are dealing with a bacterial process, a culture will help us identify the bacteria causing the inflammation in the interdigital spaces. If it's a chronic skin condition, the diagnosis is made either visually and after ruling out fungal or bacterial components, or in some cases, certain allergy tests are performed to identify connective tissue diseases, which are also performed if necessary. In general, any infectious process, whether fungal or bacterial, can easily develop into the corresponding type of eczema within one and a half to two months. If it's a fungal infection, it's mycotic eczema; if it's bacterial, it's microbial bacterial eczema. If the diagnosis is a fungal infection, fungal eczema, the first line of treatment is antifungal creams, either orally or intravenously. If it's a bacterial infection, antibiotics, creams, and ointments should be used topically. But the fight against the allergic and inflammatory process doesn't end there. Medications are needed to remove infiltration from the skin, and these are mostly standard dermatological regimens. But this isn't all there is to it. Sometimes cracks don't heal for a long time. The necessary medications that enhance regeneration in the cracked area help the body heal or scar itself. This could include methyluracil or Bepanthen, but these medications are selected, of course, depending on the stage of the inflammatory process and the stage of treatment. Ultraviolet light is often used to treat mycotic eczema and, in general, mycosis of the interdigital spaces. If this mycosis is also associated with damage to the nail plates of adjacent fingers, which are a reservoir for the fungus, then it is necessary to remove the affected areas of these nail plates and prescribe antifungal treatment.
