Semen retention is often sold online as a way to "increase testosterone." From a physiological point of view, this would need to mean a sustained alteration of the hypothalamus-pituitary-testicular axis (GnRH → LH → Leydig cells), and not just transient variations linked to sexual arousal, orgasm, sleep, stress, circadian rhythm, etc. What the evidence shows about testosterone: 1. The "7-day peak" became famous, but the study was retracted. A small trial (28 men) described that serum testosterone would peak on the 7th day of abstinence, reaching 145.7% of baseline. However, this article is marked as "Retracted article" on PubMed, with a retraction note. In other words, it cannot be used as reliable evidence to support the idea that "retaining semen increases testosterone." 2. Other studies suggest, at most, a slight increase after longer abstinence—with low robustness. A classic experimental study evaluated 10 men before and after 3 weeks of abstinence: orgasm itself did not alter testosterone, but the authors observed higher levels after the abstinence period. It is a small sample size, with a high chance of individual variability and without necessarily indicating relevant clinical gain. 3. Practical conclusion from the human literature: there is no good evidence that "seminal retention" produces a clinically consistent and lasting increase in testosterone in eugonadal men (with normal hormone production). What exists are small and heterogeneous studies, where any differences may reflect normal fluctuations (circadian and day-to-day), in addition to confounding factors (sleep, stress, training, alcohol, caloric balance, acute illness). And semen/fertility? (more solid evidence) Abstinence changes seminal parameters, which is why there is standardization in collection: the WHO recommends 2–7 days of abstinence for seminal analysis to reduce variability and make results comparable. This is about the quality/quantity of ejaculate, not about "increasing testosterone". In short, without myths: • Testosterone is mainly regulated by the hormonal axis and circadian rhythm; Interventions with the best evidence to increase T (when low/borderline) are: reduction of visceral fat resistance training adequate sleep, apnea management alcohol reduction stress management treatment of secondary causes/diseases • Ejaculation/abstinence can alter hormones such as prolactin in the post-orgasm period and vary arousal/dopamine levels, but this does not equate to a sustained “hormonal upgrade”. ATTENTION: Important to reinforce: no medication should be used without medical guidance.
