Often called a "silent cancer" because it presents with almost no specific symptoms in its early stages, it is typically discovered when the disease has already progressed to stages 3 or 4. Furthermore, even after surgery and chemotherapy, recurrence frequently occurs within about 18 months. Since the patterns of recurrence and response to treatment vary from patient to patient for recurrent ovarian cancer, a personalized treatment strategy tailored to the individual patient's condition is paramount. Recently, the range of treatments applicable to patients has become increasingly diverse, ranging from various targeted anticancer drugs to maintenance therapy and participation in clinical trials. Ovarian cancer refers to any cancer that originates in the ovarian epithelium, germ cells, or stroma that make up the ovary. However, since epithelial ovarian cancer accounts for the majority of ovarian cancers, the content I will discuss from now on will also mostly focus on epithelial ovarian cancer. Epithelial ovarian cancer, which originates in the outer layer of the ovary, easily metastasizes to the peritoneum. Therefore, although it is called ovarian cancer, it is easier to understand it as cancer that has metastasized to the peritoneum surrounding the abdomen. Because peritoneal metastasis causes ascites, it is often discovered in the form of carcinoma of the peritoneum, characterized by fluid accumulation. While most patients with advanced ovarian cancer respond to initial chemotherapy, up to 80% of patients experience recurrence. Subsequently, as the anticancer drugs gradually become ineffective, resistance develops. Consequently, despite repeated use of anticancer drugs, the progression leads to carcinoma of the peritoneum, intestinal obstruction, and drug inertia, ultimately resulting in death. Today, I intend to discuss the primary treatment for such advanced ovarian cancer, treatment methods and strategies for recurrence, and finally, clinical trials. For the primary treatment of advanced ovarian cancer, it is crucial to view surgery and chemotherapy not as separate entities but as a single, integrated package, utilizing them appropriately. The key to this treatment lies in performing high-quality surgery free from complications, initiating chemotherapy at the appropriate time, and successfully completing the treatment without interruption. Recently, treatment outcomes for ovarian cancer have been improving through maintenance therapy based on individual patient genomic analysis. The purpose of ovarian cancer surgery is to remove tumor tissue that has spread throughout the abdomen, and it is known that minimizing residual tumor tissue after such surgery leads to a better prognosis for the patient. Therefore, this surgical treatment marks the beginning of ovarian cancer therapy. The decision regarding such surgery must be made after carefully considering the patient's health status, the condition of the tumor within the abdomen, anticipated complications, and the characteristics of the chemotherapy.
