In this Transition Talk workshop, Ellen M. DiPaola helps participants take charge of healthcare planning for their young adult. Ellen introduces the Honoring Choices planning process and shares free, easy-to-use toolkits to help families get started. Learn how to create a care plan that supports both families and young adults while reflecting the young adult's wishes. You'll learn about the importance of Health Care Proxies, rights in healthcare decision-making, and the roles of Health Care Agents, guardians, and family members.
