Treatment – Part two with Dr Jane Ho

In this episode, Joel and Lani cover part two of treatment with the help of Dr Jano Ho, Paediatrician and Staff Specialist at Sydney Children's Hospitals Network (SCHN), working in the Trapeze service within the Youth & Transition Department, SCHN. They explain in more detail what it looks like to transition health care from paediatric to adult health care settings.  Chronic Conundrums has been created by four young people living with chronic illness and/or disability to create a space for conversations about their realities.
