Lecture from the series "A Date with Psychoanalytic Practice Today," organized by the Seminar of the Freudian Field of the Clinical Section of Madrid (Nucep). Since Sigmund Freud published his findings on the psychic causality of symptoms, the development of a robust and well-tested clinical approach began, one that would take into account the unconscious dimension and drives in differentiating the major structures: neurosis, psychosis, and perversion. For his part, from his earliest writings, Lacan recognized the need to consider, beyond clinical types, an unfathomable decision of being; that is, the unique reasons that operate in the personal modality of a defense against the real (which Freud called intolerable representation) and where different subjective positions for confronting existence can be recognized. However, the effectiveness of the diagnosis is contingent upon transference and the peculiar way in which the sufferer's demand is expressed, the scope of which allows us to gauge the social dimension of the symptom. Hence Jacques-Alain Miller's proposal of the epistemic category of ordinary psychosis and the consideration of autism as a separate clinical entity. How are we to read the signs of subjectivity today? A reflection on the modes of presentation, the masks, the semblances where bodies reside in search of an identity as elusive as it is precarious is essential.
