Alopecia Areata, Novidades no Tratamento – Dr. Paulo Müller Dermatologista

In 2018, we released a video addressing the new perspectives that a group of medications known as JAK Inhibitors offer for the treatment of more severe cases of Alopecia Areata. Although JAK Inhibitors were not developed for this specific purpose, the "off-label" use of these medications for the treatment of some cases of Alopecia Areata has shown very interesting results. Some patients who had no hair on their bodies (Alopecia Areata Universalis) achieved complete hair regrowth, returning to normal hair volume. Since then, several studies have been published on the subject, most with Tofacitinib (a first-generation drug), resulting in a much broader understanding of the use of these medications. However, these studies had methodological limitations and did not accurately demonstrate the efficacy rate. The novelty is that many new studies are underway, and methodologically, they are known as randomized clinical trials. This type of study prospectively evaluates patients and compares them with other drugs or a placebo, allowing for a better understanding of the efficacy and side effects of medications. Last year, a large study was published in the Journal of the American Academy of Dermatology (link below), which evaluated the use of two new JAK inhibitors, Ritlecitinib and Brepocitinib. In general, the drugs did not present intense side effects. Brepocitinib showed the best results: 65% of patients showed at least a 30% improvement 34% of patients showed an improvement greater than 90%. These more modern medications are not yet commercially available, but I believe that in the coming years they may receive approval for the treatment of Alopecia Areata. Let's keep an eye on the news!
