Virtual Annual Wellness Visit via Telehealth

Due to the recent outbreak of COVID-19, providers have had to utilize telehealth to keep the continuity of care for patients. Now, with our Annual Wellness Visit (AWV) application you can conduct an annual physical visit via telehealth. Our AWV application is integrated with an annual patient review form, fall risk assessment, cognitive assessment, PHQ1 and 9 forms, counseling forms based on patient social history and ACP forms for each state POLST and MOLST form. Utilizing telehealth for the AWV will keep patients safe, while also providing opportunities to identify candidates for the Chronic Care Management and Remote Patient Monitoring programs.
