Prolactin causes the breasts to grow and make milk during pregnancy and after birth. Prolactin levels are normally high for pregnant women and new mothers. Levels are normally low for nonpregnant women and for men. High levels of prolactin may occur due to medications, and health conditions. High prolactin develops when one of the normal prolactin-producing cells in the pituitary gland develops a mutation. High prolactin levels may have links to certain mood disorders. The treatment for high prolactin depends on its cause. Some people who have high prolactin levels but have few or no symptoms don’t need treatment. Treatment options for high prolactin include medication, surgery, and radiation therapy. Dostinex 0.5mg (kabergolin):- Cabergoline is a medication primarily used to treat disorders related to high levels of the hormone prolactin, such as hyperprolactinemia. It belongs to a class of medications known as dopamine agonists. Here's a detailed breakdown of its uses, side effects, drug interactions, and contraindications: Uses:- 1. Hyperprolactinemia: Cabergoline is highly effective in reducing prolactin levels in individuals with hyperprolactinemia, which can occur due to various reasons such as pituitary tumors (prolactinomas), medication side effects, or unknown causes. 2. Prolactinoma: It is particularly useful in managing prolactin-secreting pituitary tumors (prolactinomas), often leading to tumor shrinkage and normalization of prolactin levels. 3. Treatment of Parkinson's Disease: Cabergoline is also sometimes used as adjunctive therapy in the treatment of Parkinson's disease, particularly in patients experiencing motor fluctuations. Side Effects:- I. Nausea and Vomiting: Common side effects include gastrointestinal symptoms such as nausea and vomiting, particularly when starting treatment or with higher doses. II. Headache: Some individuals may experience headaches, which are usually mild and transient. III. Dizziness: Dizziness or lightheadedness can occur, especially when standing up quickly from a sitting or lying position. IV. Fatigue: Cabergoline may cause fatigue or drowsiness in some individuals. V. Orthostatic Hypotension: A sudden drop in blood pressure upon standing, which can lead to dizziness or fainting. VI. Psychiatric Symptoms: Rarely, cabergoline can cause psychiatric symptoms such as hallucinations, confusion, or impulse control disorders like compulsive behaviors or gambling. VII. Cardiac Valve Disorders: There have been reports of fibrotic cardiac valvulopathy associated with long-term use of cabergoline, though the risk appears to be low. Drug Interactions:- 1. Dopamine Antagonists: Concurrent use with dopamine antagonists, such as antipsychotic medications, may diminish the effectiveness of cabergoline. 2. Medications Metabolized by CYP3A4: Cabergoline is metabolized by the enzyme CYP3A4, so drugs that induce or inhibit this enzyme can affect cabergoline levels in the body. 3. Ergot Derivatives: Concurrent use with ergot derivatives is contraindicated due to the potential for additive vasoconstrictive effects, which can lead to peripheral ischemia or other vascular complications. Contraindications:- 1. Hypersensitivity: Individuals with a known hypersensitivity to cabergoline or any of its components should not use this medication. 2. Uncontrolled Hypertension: Cabergoline should be used cautiously or avoided in individuals with uncontrolled hypertension, as it can further lower blood pressure. 3. Pregnancy and Lactation: Cabergoline is contraindicated during pregnancy and lactation due to its potential effects on fetal development and lactation suppression. Additional Precautions:- 1. Liver Dysfunction: Cabergoline should be used with caution in patients with liver dysfunction, as it is primarily metabolized by the liver. 2. Cardiac Valve Monitoring: Patients receiving long-term treatment with cabergoline should undergo periodic cardiac valve monitoring to detect any signs of valvulopathy. 3. Impulse Control Disorders: Patients should be monitored for the development of impulse control disorders, particularly in higher doses or long-term use. ================================== #drasifizhar #doctor #prolactin #dostinextablet #medicinereviewandtips #medicineknowledgehindi #medicinereview
