Many people believe that cancer patients typically die in the final stages of life due to the tumor growing larger and metastasizing, eventually leading to organ failure. However, in clinical practice, we often see a paradoxical situation—imaging may not show significant tumor progression, yet the patient experiences a rapid breakdown within a short period. In this video, I want to share an important study published in Nature Medicine in December 2025. This study attempts to answer a long-standing question for frontline physicians that has rarely been systematically studied: 👉 What exactly happens to cancer patients in the final stages of their lives? The study integrated three levels of evidence: Clinical anatomy and imaging analysis Prospective blood tracking of terminally ill patients Analysis of a large independent patient database The results showed that in many terminally ill cancer patients, tumors invade large blood vessels circulating tumor cells (CTCs) proliferate rapidly before death, forming masses vascular and coagulation systems enter an irreversible state of collapse But this does not mean that "metastasis is unimportant," nor does it mean we should immediately change treatment guidelines. This is a study that helps us understand the final stages of cancer treatment, not a new guideline to immediately guide treatment.
