Treatment is necessary in the following cases: The varicocele is large, causes a lot of pain, or causes pain during exercise. When there is a bilateral varicocele. When the varicocele causes a reduction in testicular size in an adolescent. When the man has fertility problems. WHAT HAPPENS IF I DON'T TREAT A VARICOCELE? The pain may not completely disappear. Another possibility is that the testicle may gradually shrink, affecting its function. (This doesn't happen immediately; it can take months or years, and it can lead to fertility problems. Many men don't know why they can't have children, and this could be one reason.) CAN A MAN WITH A VARICOCELE HAVE CHILDREN? Yes, but not all men with varicoceles are infertile.
